Our office works with a large variety of insurance carriers and adjusters. We work directly with them prior to your first visit to ensure your care is authorized. You will not have to pay out-of-pocket for authorized services.

Our standard is to prepare and send documentation (DWC-25 and progress note) are within 72 hours of the visit.
Expedited requests can be accommodated when needed for hearings, IMEs, or utilization review deadlines. -

Please email your request to Madison Slavens at madisons@bupdr.com and include the date of service (DOS), claim # and injured worker’s full name. We will provide the DWC-25, visit note, and upcoming appointment schedule in accordance with Florida Workers’ Compensation reporting requirements. -

Typical conferences are 10–15 minutes and used to review clinical progress, work status, and treatment recommendations. Our team will confirm availability and coordinate a call time. -

Our office adheres to the Florida Department of Workers’ Compensation Fee Schedule for physician professional fees. Fees for both evaluation and subsequent treatment are processed upon the completion of the assessment and receipt of authorization from the corresponding insurance carrier.
